　　外国人在中国就业，由用人单位提出申请向其与劳动行政主管部门同级的行业主管部门提出申请，并提供下列有效文件：①拟聘用的外国人履历证明；②聘用意向书；③拟聘用外国人原因的报告；④拟聘用的外国人从事该项工作的资格证明；⑤拟聘用的外国人健康状况证明；⑥法律、法规规定的其他文件。
　　特别提示
　　在填写本格式时应当注意：
　　(1)用人单位应当说明聘用外国人的理由以及外国人的姓名、性别、婚姻状况等自然情况；
　　(2)拟聘用的外国人从事何种工作，该外国人在此之前的职业状况也要说明清楚；
　　(3)填写本表要求字迹工整，不得潦草。
